It reminds me of a scene from an episode of Married With Children. Al Bundy was relaying to his family how a fat woman entered his shoe store and asked what he'd recommend wearing for a hike in the woods. Al suggested a large sign she could hang on her back which would say, "Don't shoot. From the front, I look human."

Black bear sizes
Males — 250 lbs. (range from 125 lbs. to over 600 lbs.)
Females — 150 lbs. (range from 90 lbs. to over 300 lbs.)
Black bears are common in Washington State, and they're not a lot different in size than humans, when standing on their back feet (~5 feet to their head). So it's very possible to make a mistake.
OK, so the size is not out of the question. Do bears usually wear blue ponchos? That's what some of the articles said the woman was wearing.
It isn't unusual to have a tragic hunting accident. The Blue poncho would not have stood out with surrounding green foliage. The blue would not have triggered instant recognition to the human eye, especially in a low light condition with shading from foliage. Hunters wear bright flourescent Orange vests and hats that stand out from the blue-green spectrum.
I attended a trial of a man who shot his hunting partner while turkey hunting. The man was obviously bigger than a turkey yet the shot was still taken. Bad things happen.
