The rabbits in Night of the Lepus.
I've seen a lot in various modes. The Exorcist can make me whimper like a little girl even now. The truck in Duel is Evil. The house in The Haunting (original) and The Legend of Hell House creeps me out beyond belief.
The one that makes my skin crawl when I think about it, though, is the little girl in the The Ring (American version).
I fear Lindanumbers when she sees this thread title.
Another genre. Liberty Valence was just dangerous. So dangerous, John Wayne had to bushwhack him. Really good story. Really bad villain.

You're nails. There's a reason they put her face at the end of every "screamer" video on the internet. Maybe if I watched it now for the first time it wouldn't be the same, but at 7, that movie screwed me up pretty well.
I guess way further down on the list would be Mola Ram from Indiana Jones and the Temple of Doom. I was around 4 when that movie came out and my parents took me to see it in the theater. That whole sacrifice scene is still pretty creepy, but the effect is lost when he takes the horns off.
